Application for membership

 

If you apply for membership within 3 months of commencement of service you will be

admitted without any requirement that your membership be approved by the Committee.

Unless you are transferring from VHI / Quinn Health / Vivas you will not become entitled

to our benefits until 13 weeks (waiting period) after receipt by us of your first subscription.

However, if you are currently a member of another Health Insurance Scheme,

eg. ( VHI / Quinn Health / VIVAS ) you will have already completed the statutory waiting

periods and these will be taken into account when you join us. If your cover with VHI /

Quinn Health / VIVAS ceased more than 13 weeks prior to joining us, you will have to

undergo the waiting periods again.

 

Applications received after 3 months are subject to approval by the Committee which

usually applies futher waiting periods. Members / spouses / partners become entitled to

maternity / pregnancy benefit 52 weeks (waiting period) after receipt by us of their first

subscription (does not apply to those transferring from another scheme).

 

The following documents should be completed and returned to the Secretary at the

address shown on the application form.

 

APPLICATION FORM - It must be fully completed.

COPIES of birth certificates for all those named on the Application Form.

A COPY of your marriage certificate or completed declaration in respect of a partner.

A DOCTOR'S NOTE - regarding the current state of health of all those ( except the

                                Recruit Officer) named on the Application Form.

AUTHORISATION - This Form is your instruction to us to deduct the premium from

                             your pay. It only requires your signature and pay number and

                             we will complete the rest.
